12. Stem이 varus되지 않게 osteotomy한다. Soft tissue는 rongeur와 blade(또는 bovie)로 잘라낸다.
13. Femoral intramedullary cancellous bone을 osteotomy한다.
14. Femoral canal을 T-taper로 확인한다.
15. Template 상의 stem size를 기준으로 reaming 하면서 stem size를 결정한다. Reamer와 rasp을 차례대로 setting 해서 준다.
16. Head trial과 rasp을 제거한다. 세척하고 항생제 적신 거즈를 packing한다.
17. pulg seizer trial과 plug를 삽입한다.
18. Pulsavac으로 medullary cavity 내를 debridement하고 세척한다.
19. Pulsavac으로 세척하고 cement mixing한다.
20. Cement injection한다. Long smooth forceps, freer elevator, stem impactor를 준비한다.
21. Proximal femur까지 cement가 들어가면 Stem을 삽입한다. Stem, impactor, hammer를 준비한다. 흘러나온 cement를 제거한다.
22. Cement에서 나는 열을 식히기 위해서 세척한다. Cooling 역할
23. Packing 했던 항생제 적신 4X8거즈 제거한다.
24. Redovac(400cc)를 삽입한다.
25. Skin과 capsule을 봉합한다.
26. Compressive dressing한다.
Under G/A에서 prox.은 ASIS level에서 post edge of center of greater trochanter에 parallel하게 다시 distal은 femur shaft를 따라 10cm distal skin incision hip extension 상태에서 post dissection. prox로 gluteus maximus의 m.fiber를 direction에 따라 blunt split하고 distal로 gluteus maximus의 tendinous insertion 확인. Short ext.rotator를 exposure 한 후 knee flextion, hip I/R하고 short ext rotator cut 시행 후 capsule에 incision 후 tag suture 시행.
Bipolar component의 size는 excised femoral head의 outer diameter를 template에 measure. Femoral neck의 lat aspect와 med aspect of GT의 reaming portion을 remove하여 canal의 center를 access.
Inserted awl into the lateral most aspect of the cut surface, and then swing into greater trochanter. Begin with the smallest size broach, progressive larger broach to crush and remove cancellous bone.
Final component stem size selected, trial로 assess limb length and stability.
Plug inserted. Begin preparation of cement.
Inserted the component when the cement entered medim dough phase, typically about 6minutes after mix. Reduction 시행.
Sufficient saline irrigation was done, And antibiotic irrigation(Taulodine) was sufficiently done. Redovac(400cc) was inserted. Skin and capsule suture was done layer by layer.
Compressive dressing was done
Ⅵ. 결론
2010년 03월 19일에 Fx. intertroch femur Rt(오른쪽 대퇴 전자간 골절)으로 BHA 수술을 받으신 환자 분의 사례연구를 통해서 Fx. intertroch femur는 꼭 사고로 인한 골절이 아니라 노화로 인해서 가볍게 넘어져도 생길 수도 있고, 또한 어느 정도의 골절에 따라서 인공관절 치환술을 결정하는 지도 알게 되었다.
우리나라는 점차 고령화 사회로 되가고 있는 추세인데 그렇다면 이러한 BHA는 앞으로 더 많은 수술이 이루어질 것이라고 생각한다. 이에 대비해서 노인들은 항상 넘어지는 것도 대수롭게 생각하지 않고, 항상 주의하는 교육이 필요하다고 생각된다.
처음 실습을 수술실로 나가면서 긴장도 많이 했지만 직접 내 눈으로 수술과정을 볼 수 있었다는 것이 정말 좋았다. 책에서만 보았던 것과 실제로 보는 것은 정말로 달랐다. 수술실실습 전에는 막연하게 징그럽고 무서울 것이라는 생각도 있었지만 막상 수술실 실습을 끝마치고 나니 더 이상 우리 몸에 시행하는 수술들을 볼 수 없어서 아쉽기도 했다. 또한 신체에 대해서 좀 더 공부할 수 있어서 좋았던 실습이었다.
