lk 2-0를 traction suture을 준비한다.
⑥Sternohyoid muscle과 sterno thyroid muscle(strap muscle)을 노출 후 lateral로 retraction하며 thyroid gland를 dissection하여 mobilize한다.
Muscle을 젖혀가며 thyroid 주변의 artery, vein을 silk3/0 tie 한다.
⑦thyroid gland의 superior pole을 sharp dissection하여 vessels을 identify 한 후 thyroid gland 위에서 ligation하여 superior pole을 mobilize 한다.
Kelly 이용하여 박리하도록 돕는다.
(mosquitoes → mosquitoes → metzen baum → tie or mixter → tie → mosquito → metzenbam → tie
⑧Thyroid gland를 medial side로 retraction하면서 middle thyroid vessel과 inferior thyroid vessel을 ligation 한다.
mosquitoes, sharp pin을 주로 사용하고 metzen baum과 bovie를 준비한다.
⑨Thyroid의 lateral side를 sharp dissection하여 recurrent larygeal nerve를 identify한다.
⑩Kelly나 mosquitoes clamp로 trachea lateral side에서 recurrent larygeal nerve을 손상하지 않도록 하며 thyroid gland를 조금씩 clamping하여 resection한다.
Kelly → kelly → metzenbaum → silk 3/0 tie mosquitoes → mosquitoes → metzenbaum → silk 3/0 tie 준비한다.
Delpian lymphnode biopsy를 하기도 한다.
⑪Thyroid gland의 trachea attachment를 metzenbaum scissors나 bovie로 박리하고 isthmus portion은 kelly로 조금씩 clamping 후 resection하여 thyroid gland를 제거한다.
⑫Saline irrigation 후 bleeding 여부 확인한다.
Warm saline irrigation을 준비한다.
Tissucol 2ml를 준비한다.
(빨리 녹지 않기 때문에 사용여부를 물어보아 미리 준비한다.)
Scrub Nr.rk 2차 count를 할 수 있도록 돕는다.
⑬Wound closure
Deep cervical fascia를 interrupted vicrly 3/0를 이용해 suture한다.
Platysma muscle을 interrupted vicryl 4/0 를 이용해 suture 한다.
Skin은 steri-strip을 이용해 마무리 한다.
Muscle suture 위해 vicryl 3/0(9114)를 준비한다.
suture 후 Guardix 1.5g을 준비한다.
Subcutis suture 위해 vicryl 5/0(9105)를 준비한다.
Skin은 steri-strip을 적당한 크기로 잘라둔다.
병동에 다음 환자 준비를 연락한다.
(1차 준비)
3차 count를 돕는다.

Dressing 준비
(potadine-빵 2개 compression-elatex 3")
3차 count를 시행한다.
간호기록지를 작성한다.
3) 수술 후 간호
① 마취 깬 직후
환자의 소변량을 확인하고 비운다.
Bovie plate 부착부위를 확인하고 환자의 신체적 이상 유무를 확인한다.
체온보호를 위해 따뜻한 시트를 덮어 준다.
수술실 이동 침대로 환자 옮기는 것을 돕는다.
② 2차 연락간호를 실시하고 computer에 수술이 끝났음을 입력한다.
수술재료를 처방 낸다.
