구순열과 구개열은 일차성 구개(primary palate)나 이차성 구개(secondary palate)의 형성과정에 장애가 있어서 생기게 되는데 구순열은 태생 4-7주에 상구순이 만들어지는 과정에 장애가 있어서 발생되며, 구개열은 태생 7주-12주경 이차성 구개의 유합 과정에 장애가 있을때 발생 하게 됩니다. 원인은 유전적 요인이나 환경적 요인 어느 하나가 독립 적 으로 작용 하여 발생 하기 보다는 서로 여러가지 요인이 복합 적 으로 결합 되어 발생하게 됩니다.

입 천장이 형성되는 시기인 임신 8주에서 12주 사이에 입 천장이 제대로 형성되지 못하여 생긴다, 갈라진 구개열은 입 천장의 융합과정중에서 언제, 어떻게 융합과정에 이상이 생겼는가에 따라서 갈라진 변형의 정도와 위치가 다양 합니다
아기의 입술과 입 천장이 만들어 지는 시기는 임신 첫번째 3분기(임신 첫 3개월)이며. 대부분 정상아 에서는 입술의 오른쪽 편과 왼쪽 편이 서로 만나 융합됩으로서 정상적인 두 개의 수직선(인중선)이 생기게 된다. 이 융합은 임신 4주에서 6주 사이에 일어나며, 만일 이 유합이 완전치 못하게 일어 나면 구순열을 가진 아이가 태어나게 괸다

feeding difficulties
Feeding difficulties occur more with cleft palate abnormalities. The infant may be unable to suck properly because the roof of the mouth is not formed completely.
ear infections and hearing loss
Ear infections are often due to a dysfunction of the tube that connects the middle ear and the throat. Recurrent infections can then lead to hearing loss.
speech and language delay
Due to the opening of the roof of the mouth and the lip, muscle function may be decreased, which can lead to a delay in speech or abnormal speech. Referral to a speech therapist should be discussed with your child's physician.
dental problems
As a result of the abnormalities, teeth may not erupt normally and orthodontic treatment is usually required.
