ert GI bleeding(hematemesis, melena, hematochezia)에 대한 개념으로, routine endoscopic studies(EGD & colonoscopy로 90%의 GI bleeding이 진단된다)에서 원인(source)이 발견되지 않는 출혈을 일컫는다. Occult GI bleeding은 laboratory tests나 대변검사에서 진단되는 출혈이다.
원인으로 소아에서는 Meckel's diverticulum이 가장 흔하고, 성인에서는 angiodysplasia가 75%, 악성종양이 10%를 차지한다. 이외에도 Crohn's disease, infectious enteritides, NSAID-induced ulcer and erosion, vasculitis, ischemia, varices, diverticula, intussusception 등이 원인이다.
11. Small bowel perforation
가장 흔한 원인은 iatrogenic injuries(특히 ERCP, endoscopic sphincterotomy)이다. 진단은 CT scanning이 가장 좋으며, 치료는 정도에 따라서 관찰하는 경우부터, 개복수술까지 다양하다.
12. Chylous ascites
Intestinal lymph인 milky or creamy triglyceride-rich peritoneal fluid가 복강내에 고이는 것을 말한다. 원인으로는 복강내 악성종양, 간경변, tuberculosis, filariasis 등이다. 또한 복부나 흉부 수술(abdominal aortic aneurysm repair, retroperitoneal lymph node dissection, inferior vena cava resection, liver transplantation)이나 trauma가 원인이다.
진단은 복수천자를 통하여 cloudy & turbid한 chyle을 확인하는 것이나, 금식 시에는 색깔이 맑을 수도 있다. Triglyceride가 110 mg/dL 이상 시에 확진이 된다. CT, lymphangiography, lymphoscintigraphy 등이 진단 혹은 수술계획을 세우는데 도움이 된다.
치료로는 원인질환의 교정과 함께 high-protein, low-fat(medium-chain triglyceride) diet를 한다. 심하면 금식과 더불어 TPN을 하고, octreotide가 림프액의 분비량을 줄일 수 있다. 내과적 방법으로 60% 정도에서 치료가 되나, 30% 정도는 수술이 필요하다. 원인질환에 대한 처치가 불가능한 경우에는 peritoneovenous shunting을 하기도 하나 합병증이 높다.
13. Intussusception
성인에서는 leading point가 있는 경우가 대부분이고, 악성종양에 의한 것이 1/2이다. Roux-en-Y gastric bypass에서 retrograde intussusception이 leading point 없이 발생하기도 한다.
