Pulp와 periradicular ts.에 생기는 대부분의 병변은 직간접적으로 미생물과 관련되어 있으므로 근관병소와 관련되어 있는 세균에 대해 아는 것은 질환의 진행에 대한 이해와 확실한 치료의 근거를 형성하는데 있어 중요.
이 chapter에서는 pulp와 periradicular ds.와 관련된 bacteria, infection control, 보조적 항생제 치료를 포함하는 근관감염의 치료에 대해 설명.
*Bacteria associated with endodontic infection
1.Terminology
colonization ; living host에서 세균이나 다른 미생물의 집락형성. growth에 적합한 biochemical, physical condition하에서 또한 세균을 죽일 억제효소가 불충분할 때.
normal oral flora ; host와 symbiotic relationship인 경우에서의 permanent colonization. 대개는 유익한 현상에 관여하나 opportunistic pathogen이 될 수도 있다.
pulpal & periradicular pathogen은 oppotunistic pathogen이 치수와 근첨부 조직을 감염시킬 때 발생
infection ; host에 damage를 주어 clinical sign & sx.을 유발하게 된다.
Pathogenicity ; 특정 숙주에서 질환을 일으킬 수 있는 세균의 능력
virulence ; 정의된 조건하에서 숙주에서 나타나는 병원성의 정도
endodontic infection의 발생 단계
microbial invasion→colonization→multiplication→pathogenic activity
