2. Follicular carcinoma
the low-grade encapsulated (microinvasive) follicular carcinoma
Dx는 오직 total lobectomy and isthmusectomy후의 multiple slides검사로만 가능하다.
total thyoidectomy
① 종양의 크기가 4cm 이상이고, frozen section에서 follicular neoplasm시
② 종양의 크기가 1cm 이상이고, frozen section에서 equivoval 할 때(microinvasion의 risk 때문)
③ 종양의 크기가 1cm 이상이고, angioinvasion, more than minimal capsular invasion시
:higrl-grade angioinvasive follicular carcinomas : total thyroidectomy & I131 Tx. for metastatic
disease.
3. Hurthle cell carcinoma
- total thyoidectomy and ipsilateral cetral neck dissection
- enlarged jugular nodes(+)시 modified neck dissection
4. Undifferentiated thyroid carcinoma
- FNAB로 진단 후 combination chemotherapy and radiation therapy
- CT scanning으로 extent & the potential operability 결정
- tracheostomy가 필요할 수 있다.
5. Lymphoma
- Dx 후 staging으로
① local : radiation therapy
② extensive or relapse:chemotherapy
6. Postoperative treatment
- I131 radioactive iodine
- thyroid hormone 투여
① hypothyroidism 방지
② TSH 분비 억제로 종양성장의 억제
-extemal radiation의 적응증
① Lymphoma
② undiffereutiated thyroid carcinoma
③ palliative Tx. with unresectable cancers
-chemotherapy
:3rd line therapy
doxorubicin, 5-FIJ, MTK, actinomycin D, cyclophosphamide 등
