❖ Definition

　• Persistence of the fetal circulatory pattern of RL shunting through the PDA and Foramen ovale after birth is due to excessively high PVR

❖ Occurs in term and post-term infants.

❖ Incidence
　• 1/500-1,500 live births

PPHN은 출생 직후 정상적으로 이루어져야 할 Pulmonary vascular resistance의 감소가 이루어지지 않고 High PVR 상태를 유지하여, Fetal circulation과 같이 Foramen ovale와 Ductus arteriosus를 통해 RL shunt가 일어나는 경우를 말합니다.

주로 term과 postterm infant에서 발생하며 발생빈도는 출생아 500-1500명 중 1명의 비율로 다양합니다.

PPHN은 특발성으로 나타날 수도 있지만 다음과 같은 Predisposing factor에 의해 이차적으로 나타날 수도 있습니다. Birth asphyxia, Meconium aspiration pneumonia, Early-onset sepsis, Respiratory Distress synd., Hypoglycemia, Polycythemia, 산모의 NSAIDs 사용으로 인한 자궁내 동맥관 조기폐쇄, 임신 후반기 모체의 SSRI 복용, 횡격막 탈장, amniotic fluid leak, 양수 과소증, 흉막 삼출 등으로 인한 폐형성 부전 등이 해당됩니다.

몇몇 PPHN환자에서는 plasma arginine 수치와 Nitric oxide metabolites 농도가 낮으며, carbamoyl phosphate synthase gene의 polymorphysm이 있는 경우가 있는데 이러한 소견은 NO 생성에 defect가 있음을 암시하기도 합니다.
