는 사람 은 RBC에 glutathione level을 적절히 유지할 수 없다.
sulfhydryl group은 산화되고 RBC내에 축적되어 Heinz body를 형성
① G6PD deficiency
가장 흔한 선천적 shunt defect이다.
G6PD gene은 X chromosome에 위치
deficiency state는 sex-linked trait
대부분의 female carrier는 무증상, deficient cell의 많은 부분은 male hemizygotes
hemolytic episodes는 viral & bacterial infection에 의해 촉발되는데 기전은 알려져 있 지 않다. 그 외에 drug & toxin이 용혈을 일으킴

G6PD deficiency 환자에서 용혈을 유발하는 약물
② 임상 및 Lab 특징
oxidant stress에 노출된 후 수시간 내 acute hemolytic crisis를 경험
hemoglobinuria & peripheral vascular collapse를 일으킴
노후된 RBC만 급격히 파괴되므로 oxidant가 지속하더라도 hemolytic crisis는 자연회복
G6PD deficiency 진단은 아프리카 혹은 지중해 남자에서 acute hemolytic episode가 있을 때 고려
enzyme activity나 deficiency effect를 평가하는 검사로 진단 할 수 있지만 검사는 false-negative result가 있을 수 있음
③ 치료
흔히 자연 회복되며 특별한 치료는 필요치 않다.
splenectomy는 도움이 되지 않는다.
hemolytic episodes의 예방이 최선이다. 감염은 즉각 치료해야 하며 oxidant drug & favabeans(잠두)에 폭로될 위험에 주의해야 한다.
oxidant drug을 사용하는 아프리카 혹은 지중해 사람은 G6PD deficiency에 대해 screen 하여야 한다.
(3) Other enzyme defects
pyrimidine 5'-nucleotidase deficiency: mRNA가 대사되지 못하여 RBC내 coarse
basophilic stippling초래
3) Hemoglobinopathies(혈색소병증)
겸상적혈구 질환은 선천성 용혈성 빈혈 중 중요한 형을 이룸
흔하지는 않으나 불안정한 혈색소 변이체의 유전에 의해 용혈이 발생
