, sphenoid wing, olfactory groove등에 잘 생긴다.
Foster-Kennedy syndrome
· olfactory groove, 혹은 medial sphenoid ridge에 생긴 meningioma로 ipsilateral optic atropy, anosmia, contralateral papilledema가 생기는 것.
· surgical resection만으로도 예후가 좋다.
Posterior fossa tumor
소아 후두와 종양
· 소아기의 뇌종양중 60%를 차지한다. medulloblastoma와 posterior fossa astrocytoma가 대표적
Tumors of the cerebellopontine angle
Acoustic neuroma
· 이 부근에서 발견되는 종양 중 가장 흔하다.
· 8th nerve의 vestibular portion에 생긴다.
· tinitus, hearing difficulty, facial numbness가 주증상.
Pituitary tumor
· functioning tumor와 non-functioning tumor로 나눌 수 있다.
증상
· Cushing syndrome - ACTH이상
· acromegaly - growth hormone이상
· amenorrhea, galactorrhea, infertility - prolactinoma의 triad
· Nelson's syndrome : hyperpigmentation and pituitary tumor
· non-functioning tumor의 경우 mass effect로 visual symptom-bitemporal hemianopsia를 나타낸다.
Pineal region tumors
· midbrain compression으로 인한 ocular manifestation이 먼저 나타남.
Parinaud's syndrome
· paralysis of conjugate upgaze, ocular convergence의 마비, pupillary constriction의 소실
CSF pathway를 눌러 hydrocephalus, pineal gland 기능마비로 percocious puberty등이 나타남.
Germ cell tumor는 여러가지 tumor marker에 의한 진단이 가능하다.
· germinoma : PLAP
· choriocarcinoma : HCG
· embryonal carcinoma : HCG, AFP
· endodermal sinus tumor : AFP
Metastatic brain tumor
primary site : 남성의 경우 lung, colon, kidney 등이 있고, 여성의 경우는 breast, lung, colon and melanoma가 있다.
방사선 소견 : ring enhancement, surrounding edema
2개 이하일 경우 수술을 하기도 하나 대체로 2개 이상일 경우는 RT, chemotherapy등으로 증상만 덜어주는 치료를 한다.
