본문내용

여경로, 약제 투여시 부작용 및 이의 예방을 위한 처치 등을 이해한다.
나. 내용
(1) 항암화학요법의 목적
1) 일차 종양이 제거된 후 원발 병소로부터의 전이 방지 및 전이 병소 제거한다.
2) 수술이나 방사선치료의 전단계로 원발 병소의 크기를 줄인다.
3) 수술이나 방사선 치료 후 잔류 종양의 제거한다.
4) 원발 종양 또는 전이된 종양 치료에 있어 방사선 치료의 효과를 상승시킨다.
5) 혈액종양 질환의 관해를 유도하고 이를 유지한다.
6) 종양의 크기를 줄이거나 조절함으로써 삶의 질을 개선한다.
7) 잔류질환의 증상을 완화시킨다.
(2) 복합 항암요법 (combination chemotherapy)의 중요성
1) 단일제제 사용시 발생할 수 있는 약제 내성을 예방한다.
2) 이미 내성을 가진 종양의 경우 이를 극복한다.
3) 다른 작용을 하는 약제의 동시 사용시 약제의 상승효과를 얻는다.

(4) 항암제의 혈관외 누출
1) 예방 ; 가능한 모든 주의를 기울이는 것이 가장 좋은 방법이다.

2) 일반적인 처치 방법

2 aspiration of any residual drug remaining in the tissue should be attemped
3 local measure ; ice pack or local heat apply
subcutaneous injections of saline, steroids, or bicarbonate may be useful.
(5) 기타 합병증
1) 종양용해 증후군 (tumor lysis syndrome)

close follow up of body weight, blood pressure, urine output
close follow up of urinalysis(SG & pH), BUN, Cr, Uric acid, Ca/P & other electrolytes
3 혈액 투석의 기준
K+ > 6 mEq/L with rising Cr and urine flow< 60ml/m2/hr
uric acid > 10mg/dL with rising Cr and urine flow< 60ml/m2/hr
Cr > ×10 of normal
serum phosphrous > 10mg/dL or rapidly rising
volume overload
symptomatic hypocalcemia with hyperphosphatemia
(6) 호중구 감소증과 발열
1) 발열시 검사와 항생제의 투여
