. Meniscectomy
1) 준비물
Arthro set, MP, MT, Knee pack, Arthrocare, 4mm 30° scope, Arthro forcep set, arthrocare, 4mm sleeve, output troca, suction tube, shaver tip(4mm smooth/ aggrresive), leg sleeve, EB, light handle, no.11 blade, pen, nylon 3-0
2) Procedure
prep
soap → alcohol → povidone
drap
Sheet #2, skin towel & towel clamp → U-drap → leg sleeve → 대공 → EB(squeezing)
portal 확보
1. no.11 blade + no.3 knife handle(칼날이 하늘을 향하게) → mosquito str. → 4mm sleeve → troca 빼고 scope 삽입
2. spinal needle + mass → mosquito str. → output troca
3. working protal 하나 더 만들어서 probe, shaver(radius)로 정리
meniscectomy
Arthro forcep set에서 forcep 줌(환자 상태, 필드 각도에 따라 다양하게)
shaver(aggressive), arthrocare로 지저분한 부위 정리
suture
nylon 3-0
dressing
povidone 직접 손에 닿지 않게 kelly 등에 물려서 준다 → gauze → cotton roll → EB
3) 상차림
5. Patella Fx.
1) 준비물
OS set, MP, MT, PREP, GP, Knee pack, OS 베개, recudtion clamp(중, 대), pin cutter, steinman pin(thread X), k-wire, 1.8 k-wire, wire needle holder, roll wire(21 or 22G), inpactor, wire passor, micro currette, spoid, bovie, suction tube, 일회용 sheet, leg sleeve, EB, cotton roll, light handle, no.15 blade, gown, glove
2) Procedure
prep
soap → alcohol → povidone
drap
leg sleeve → assist gowning → 다리 아래 sheet → 몸통 sheet → skin towel & towel clamp → U-drap → 대공 → EB
marking
squeezing용 EB 풀고 bandage scisor로 stockinet 자름 → marking pen으로 marking. 무릎 아래 OS 베개 두고 약간 굴곡시켜 준다.
dissection
no. 10 blade or no. 15 blade, adson teeth, metzem, tissue forcep, self rectractor #2, freer, perio로 골절면이 있는 곳까지 vertical 하게 dissection 해 내려감
irrigation

reduction
reduction clamp(대) #2 → C arm check
(lateral로 찍을 때 일회용 sheet로 contamination 방지)
K-wire 고정
K-wire(1.8) drilling → roll wire 21G 건네줌 → wire passor로 k-wire 끝을 잡고 twist → pin cutter로 매듭 위쪽을 cut(mosquito로 잡아줌)
→ 튀어나와 있는 wire 매듭을 impactor와 mallet으로 patella로 넣어줌 →
위, 아래로 튀어나온 k-wire는 자르고 튀어나온 부분은 impactor로 밀어넣어줌
suture & dressing
povidone + kelly → dry gauze → cotton roll → EB → immobilizer
6. MBO (Modified Brostrom Operation)
1) 준비물
OS set, MP, MT, PREP, GP, Jar forcep, drill(anchor 사용시), drill bit, guide, leg stockinet, EB 6inch #2, cotton roll 6inch #2, marking pen, blade no.15 #2, bovie, suction tube, 50cc syr. #2, bovie tip(blade type), C-arm cover, light handle, gown, U-drap, glove, vicryl 1, vicryl 3-0, nylon 3-0, 1.8 k-wire, frence needle, sking arm needle, ethibond 2-0(ligament), vicryl 3-0, 4-0(medial), vicryl 2-0(lateral)
2) Procedure
prep
soap → alcohol → povidone
drap
skin towel & towel clip → sheet #3(다리 아래 #1, 반대쪽 다리 위 #1, 몸통 #1) → U-drap → 대공 → sheet
marking
leg stockinet 자르고 marking
incision
blade no. 15
dissection
senn retractor로 벌리면서 bovie, tenotomy. adson teeth로 dissection함.
rongeur, nasal freer, currette으로 주변 조직 정리
tie
capsule과 ATFL 확인 후 ATFL과 CF(calcaneofibular) ligament suture.
- drill로 anchor 들어갈 길 내기 → anchor 넣고 mallet으로 고정 → ethibond 2-0, mosquito로 잡고 cut
suture
필요 시 silastic drain 삽입
vicryl 3-0(muscle), nylon(skin)
dressing
wet gauze → terra oint → dry gauze →short leg splint
